Please provide at least 48-hours’ notice by email or phone if you are unable to attend your appointment. The full cost of the session will be charged in the event of a missed appointment or late cancellation (within 48 hours). Dr. LaFleur may waive the fee in the event of an unavoidable absence, such as inclement weather, family emergency, or illness. Most insurance carriers do not cover or reimburse for missed sessions.
If you arrive more than 15 minutes late to a session, Dr. LaFleur will consider it a missed appointment and charge you for the full cost of the session. If, because you arrive late or leave early, Dr. LaFleur is unable to bill insurance for the full session length that was scheduled, you will be billed for the difference between the two session fees. You will be personally responsible for paying missed appointment or late arrival fees as most insurance carriers do not cover or reimburse them.
Dr. LaFleur will similarly make every effort to attend scheduled appointments on time and will provide advanced notice, whenever possible, if there are needed changes to your schedule.
You have the right to receive a “Good Faith Estimate” explaining how much your care at Stony Run will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items and services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least one (1) business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.
Dr. LaFleur is also available to answer your questions and field requests as they pertain to Good Faith Estimates and insurance more generally.
It is important for every patient to achieve meaningful change. In the event that Dr. LaFleur does not provide the right type or level of support, she will help you find a better fit.
For treatment to be effective, patients and families must maintain regular attendance and follow treatment recommendations. Dr. LaFleur may institute specific attendance or treatment expectations to support you. If you or your family member does not make progress in treatment, or experiences challenges attending sessions regularly or following treatment recommendations, Dr. LaFleur will provide you with referrals so that you can obtain more appropriate treatment and support.
You will not be considered an active patient if (1) you have missed two consecutive sessions and have not contacted your clinician to reschedule within one week of your last scheduled appointment or (2) you have not seen Dr. LaFleur for more than one month and do not have future appointments scheduled.
If you are no longer an active patient but would like to schedule additional appointments with Dr. LaFleur, you may request one. There may be delays until Dr. LaFleur is available to add an appointment or to see you regularly as an active patient. In addition, you may contact Dr. LaFleur for referrals at any time or contact one of the referrals provided in the Informed Consent you signed at the start of treatment.
Dr. LaFleur takes patient privacy seriously and will discuss her policies with you and provide additional documentation of them.
